Beyond wellness care

Over the years, we have worked with countless individuals and families navigating the challenges of aging, recovery, and maintaining independence at home. One of our least favorite experiences is working with families that could have prevented unforeseen situations.

One thing has become clear: many of the biggest issues, such as falls, hospitalizations, and difficult care decisions, could be avoided with the right planning ahead of time.

With the rising cost of Assisted / Independent Living Facilities and growing concerns of lackluster care, the process can be daunting.

Our Aging-in-Place Assessment was created to fill that gap.

When is the right time to step in?

Is the home still safe?

Are stairs becoming a hinderance? Are there throw rugs in the house? Are pets becoming a nuisance?

Assistance needed?

How can I get help? Is an aide something my insurance will pay for?

What’s Missing?

I think I have my bases covered, but what have I not thought of? What’s missing from my plan?

Who to call?

I think I may know what to do, but what if my doctor doesn’t answer? Do I need to go to the ER every time?

This is a 3-4 hour, in-home consultation where we evaluate the living space, the individual’s mobility, and support system to provide a clear, personalized plan to improve safety and independence.

We leave no stone left un-turned, and you will feel at ease with a plan moving forward, and rest assured that you or your loved one feels safe and secure in their home.

What is an aging-in-place assessment?

A CLINICALLY informed approach you won’t find elsewhere

Unlike traditional senior services, this assessment is grounded in physical therapy and wellness expertise.

We don’t just look at the home, we evaluate how your loved one moves within it.

This allows us to identify risks and solutions that are often missed including:

  • Subtle balance deficits

  • Improper use of Assistive Devices

  • Hidden fall risks in every day movement

The result is a more accurate, practical plan your family can actually use.
